Enrollment for new students (grades 1-5):

Registration and important forms:
The following forms must be completed for school registration:

• Enrollment Form - COMPLETED by parent
• Emergency Contact Form - COMPLETED by parent
• Health History Form - COMPLETED by parent
• Private Physican's Physical Information
• Record of Immunizations (Required by Law)
• Proof of Residency (Tax receipt/mortgage/rental/utility)
• Birth Certificate- An ORIGINAL Birth Certificate from the Town/City Hall in which your child's birth is registered must be presented. Hospital or Baptismal records are not acceptable. The original birth certificate will be returned to you.

If you are new to Arlington, and may not be on the census list, please call the Bishop School, 781-316-3792 to be placed on our mailing list.

No fall assignments will be made until all these requirements are met!

In order that we may adequately prepare for all incoming students, please notify this office as soon as possible if you do not intend to register your child or are registering at a private school, moving, etc. If you have any questions, please call Mrs. Gonsalves at 781-316-3792.
