PART TIME EMPLOYMENT

Please complete the appropriate forms below and return them in person to the Payroll Department located in the Business Office, 869 Massachusetts Avenue, Arlington, MA, 02476.

U.S. Dept. of Justice, Emp. Eligibility Verification

CORI Request Form

Social Security Statement

• ICMA Enrollment Form


Personnel / Emergency Contact Form




Along with the completed forms, please bring with you the following:

  1. A state-issued driver's license or state-issued I.D. card with a photograph.

  2. An original social security card OR a birth certificate issued by the state, country, or municipal authority bearing a seal or other certification.

  3. In lieu of #1 and #2 you may bring in a United States Passport OR Certification of United States Citizenship OR a Certificate of Naturalization.
