Please complete the appropriate forms below and return them in person on the date you are attending the payroll orientation or to the Payroll Department located in the Business Office, 869 Massachusetts Avenue, Arlington, MA, 02476.

HEALTH INSURANCE ENROLLMENT FORMS

NOTE: ALL INSURANCE DECISIONS MUST BE MADE AND SUBMITTED WITHIN 10 DAYS OF EMPLOYMENT

All health insurance forms are available under the Benefits Tab in the Human Resources section of the website.


Along with the completed forms, please bring with you the following:

  1. A state-issued driver's license or state-issued I.D. card with a photograph.

  2. An original social security card OR a birth certificate issued by the state, country, or municipal authority bearing a seal or other certification.

  3. In lieu of #1 and #2 you may bring in a United States Passport OR Certification of United States Citizenship OR a Certificate of Naturalization.
