Home and Hospital Instructions
In accordance with 603 CMR 28.03(3)(C), the Arlington Public Schools provides an instructional program for any student who, in the judgment of the student’s physician, will have to remain at home or in a hospital for a period of not less than fourteen days. As soon as it is known that the student will be absent from school, the Home or Hospital Instruction program may be authorized.

Life Threatening Allergy Policy
Allergic reactions can span a wide range of severity of symptoms. The most severe and potentially life threatening reaction is anaphylaxis. This protocol is to be used for students who are at risk for anaphylaxis and in circumstances where a previously undiagnosed life-threatening allergic response occurs.

Medication Permission Form for Health Care Providers and Parents
Under Massachusetts General Laws (M.G.L.) chapter 112, 80B, a licensed nurse must have a medication order from a physician, dentist, nurse practitioner, or physician’s assistant in order to administer any medication, whether it is a prescription drug or over-the-counter medication.

The Arlington Public Schools provides education to preschool through twelfth grade students in seven elementary schools, one middle school and one comprehensive high school. Private Physician Exam Form
Registration
Kindergarten registration requires:
- Physical exam form from your child’s primary care provider. This exam must be within 12 months prior to the start of school.
- Up to date immunizations per state guidelines. If your child’s immunizations are not up to date, the school nurse will contact you and let you know what is needed prior to starting school. Your child can not start school without appropriate vaccines. There are three exemptions for lack of vaccinations which are permitted per state mandate. a. McKenney-Vento act in which your child is considered homeless, b. Medical exemption due to a medical condition or treatment that would compromise your child’s immune system, c. Religious exemptions. You as the parent or guardian, would need to fill out a yearly religious exemption form claiming that your religion does not allow for vaccinations and if there is any outbreak in your child’s school related to the unvaccinated situation, your child will have to leave school until there is no threat of disease. Philosophical exemptions are not permitted under state mandates.

Lastly, we ask parents or guardians to communicate with their school nurses if your child has any medical conditions requiring nursing assessment during the school day such as but not limited to any serious allergies requiring epi-pens, or if your child will be needing medications while in school.
