New Coaches/Sports

Please complete the appropriate forms below and return them in person to the Secretary in the Athletic Department. Please remember to bring the appropriate identification for your U.S. Dept. of Justice, Empl. Eligibility Verification when returning the forms.

Along with the completed forms, please bring with you the following:

1. A state-issued driver's license or state-issued I.D. card with a photograph.
2. An original social security card OR a birth certificate issued by the state, country, or municipal authority bearing a seal or other certification.
3. In lieu of #1 and #2 you may bring in a United States Passport OR Certification of United States Citizenship OR a Certificate of Naturalization.
