Required Documents for Enrollment

Proof of Age – Birth Certificate or Adoption Certificate

Proof of Parent/Guardian/Caregiver Identity – provide one of the following:

Proof of Residency – provide one of the following:

Deed or Rental Lease Agreement (showing names, dates, and signatures), Quitclaim Deed, Mortgage statement, Current Property Tax Bill, or Town-issued Water/ Sewage Bill.

We cannot accept Purchase and Sale (P&S) agreements as proof of residency. If you are not on the Deed/ Lease. A notarized letter from the homeowner must be accompanied by a copy of the Deed/ Lease.

The lease must be current and fully executed; expired leases cannot be accepted.

Proof of Occupancy – provide one of the following documents dated within 60 days:

Utility Bill (electric, oil, gas, solar statement) or

Confirmation of Service Notice/ Service Agreement/ New Customer Letter (for those who recently moved and have not yet received a bill)

Bank statements, phone bills, internet bills, excise tax or income tax documents are NOT accepted as Proof of Residence or Occupancy.

If utility costs are included in the rent, please provide a notarized letter from the homeowner.

Student Physical Exam Records – Must be within the past 12 months.

Please submit updated physical exam forms in the PowerSchool Portal.

Physical Exam (All Students)
All students must have a physical exam on file dated within 12 months of the school start date. The Massachusetts School Health Record document is the preferred form for submitting this information. If your child’s doctor or hospital does not use this form, other official medical documentation is accepted.

Immunizations (All Students)
A complete and legible copy of your student’s immunization records must be submitted.

Required Screenings In Addition to a Physical For Kindergarteners:

Vision Screening
Provide proof of a vision screening completed within the past year.
Lead Level
Documentation of a lead level test completed at any time is required.

Student Immunization Records

IEP or 504 Plan Documents (if applicable)

Transcripts/Progress Reports

Other Important Documents (if applicable)
