Registration Requirements

When registering for school, there are state-mandated health regulations that must be adhered to in order for students to enroll.
 
 
Kindergarten Registration Requirements:
  1. Physical examination form from your student's health care provider. This exam must be within 12 months prior to the start of school.
  2. Up-to-date immunizations in accordance with state guidelines. If your student's immunizations are not up to date, the school nurse will contact you and let you know what is needed prior to starting school. Your child can not start school without appropriate vaccines.*
  3. A lead screening on file from your student's health care provider.
  4. A vision screening on file from your student's health care provider, including stereopsis (depth perception). The vision screening must be within 12 months prior to the start of school.
 
Registration Requirements for Grades 1-12:
  1. Physical examination form from your student's health care provider. This exam must be within 12 months prior to the start of school.
  2. Up-to-date immunizations in accordance with state guidelines. If your student's immunizations are not up to date, the school nurse will contact you and let you know what is needed prior to starting school. Your child can not start school without appropriate vaccines.*
 
*There are three situations in which children not appropriately immunized may be admitted to school:
    1. A medical exemption is allowed if a physician certifies that immunization is medically contraindicated. This must be renewed annually at the start of the school year.
    2. A religious exemption is allowed if a parent or guardian provides a written statement that the immunization conflicts with their sincere religious beliefs. This should be renewed annually at the start of the school year.
    3. The Every Student Succeeds Act (which reauthorizes the McKinney-Vento Act) protects unimmunized or partially immunized homeless children and children in foster care from being denied entry to public schools if they do not have immunization records. These children are not protected from exclusion in the case of an outbreak.
Philosophical exemptions are not permitted under state mandates.
 
 

Lastly, we ask parents/guardians to communicate with their school nurse if their student has any medical condition(s) that require nursing assessment/care during the school day. This includes, but is not limited to, life-threatening allergies, asthma, diabetes, seizure disorders, etc., or if your student will need medication administered during the school day. Please see the appropriate forms to be filled out by your student's health care provider, if applicable.
