Registration Requirements

When registering for school, there are state-mandated health regulations that must be adhered to in order for students to enroll.
 
 
Kindergarten Registration Requirements:
  1. Physical examination form from your student's primary care provider. This exam must be within 12 months prior to the start of school.
  2. Up-to-date immunizations in accordance with state guidelines. If your student's immunizations are not up to date, the school nurse will contact you and let you know what is needed prior to starting school. Your child can not start school without appropriate vaccines.*
  3. A lead screening on file from your child’s provider.
 
Registration Requirements for Grades 1-12:
  1. Physical examination form from your student's primary care provider. This exam must be within 12 months prior to the start of school.
  2. Up-to-date immunizations in accordance with state guidelines. If your student's immunizations are not up to date, the school nurse will contact you and let you know what is needed prior to starting school. Your child can not start school without appropriate vaccines.*
 
*There are three exemptions for lack of vaccinations which are permitted per state mandate:
      1. McKenney-Vento act in which the student is considered homeless
      2. Medical exemption due to a medical condition or treatment that would compromise the student's immune system
      3. Religious exemptions. The parent/guardian would need to fill out a yearly religious exemption form claiming that their religion does not allow for vaccinations, and if there is any outbreak in your student's school related to the unvaccinated situation, your student will have to leave school until there is no threat of disease.
Philosophical exemptions are not permitted under state mandates.
 
 

Lastly, we ask parents/guardians to communicate with their school nurse if their student has any medical condition(s) that require nursing assessment/care during the school day. This includes, but is not limited to, life-threatening allergies, asthma, diabetes, seizure disorders, etc., or if your student will need medication administered during the school day. Please see the appropriate forms to be filled out by your student's health care provider, if applicable.
